As we welcome the New Year 2026 and reflect on 2025, I would like to thank Dr. Ramdas for arranging my transfer from Carewell Medical Center to Chatham Hills Subacute Care Center for Orthopedic Rehabilitation on 11/25/2025. From the time of admission to my discharge 12/13/2025 I was treated with respect, and compassion from an extraordinary team who ensured that not only was my medical needs attended to (recovering from broken Tibia right knee) but my concerns that "I would fall behaind in my studies as a graduate student" was not ignored but met with accomodation by staff and administration to accomodate my online studies.
And so I say with heartfelt gratitude and appreciation to Alex/Admin, who took the time to explain the "ins and outs" of Medicare and the services provided, my LPN - Kesha T, RN - Haley, who worked with the "Nursing Aides, e.g., "YO" and Tamara, in ensuring that my medication and assistance as needed, were attended to. To the Social Workers, Saba/Cynthia - arrangements for my aftercare with Atlantic Visiting Nurse and the "AWESOME" Physical Therapy Team: Leslie, Gabby, Spencer, Charlie, Tammy, Romeo, Rachel - Dir. Victor ensured I left for home with my much-needed medical supplies. To the Kitchen staff, "Chef," and the director who came by and ensured that the menus I selected were served on time and prepared deliciously. To the night crew and staff who checked in periodically to ensure that "I was resting well." To the auxiliary staff and cleaning crew who ensured that my room was cleaned and "germ-free." And so, Thanksgiving away from home was not as "lonesome" as I imagined. When my sisters, Cass & Shawn, came, they remarked not only on the cleanliness of the facility but also on the professionalism of the staff, starting with the Receptionist.
Chatham Hills, you have an extraordinary team, and I would recommend and support any referrals to your facility. I wish you continued success with your dedicated healthcare professionals. Leslie, may you have a peaceful delivery and be blessed with a healthy baby.
And "YES," I successfully "A-ced" my exams.
HAPPY NEW YEAR.
God's speed, Blessings, good health, and Prosperity to all.
Chatam Hills subacute care center has been a home for my lady/client for more than a year and I have been going there everyday to take care of her, keep her company . I can say she’s happy of the care the staff has been giving her ,she loves the back massage she’s getting every week and the entertainment. The attention she’s been getting from the staff makes her feel valued.
She loves what they do everyday.
The activity staff help the residents stay busy, and they ensure everyone participates in the daily activities. Alisson is the best! I am impressed by her commitment to going the extra mile.
I’ve been coming to Chatham Hills Subacute for a year now as a private aide. The nursing staff and CNA’s on the Nursing Home side do a great job of doing care for the residents. Some days are crazier than other days but they try to stay on top of it. Even the cleaning ladies do a good job of keeping the south side clean. Kudos to the activity crew on a job well done when it comes to activities, entertainment and decorations. Four stars because there is always room for improvement.
I’m here as a Chatham Hills Employee and also a patient’s family. Just before the Thanksgiving holiday my mother in law who I love love love took a tumble at home which caused her blood pressure to drop, she also had dizziness, confusion and weakness. During her hospitalization she was recommended to go to rehab for her current issues but more importantly for an untreated condition of lymphedema in her legs which was the cause of her fall (legs so large she was not able to move around her apartment and limiting her mobility she’s still undergoing her therapy but as a family member I’m amazed at how my team has handled her condition from day one. Jannell our DON, Christine, Unit manager, our Rehab team, our Medical Director Dr Prager, our Cardiologist Dr Saeed, our CNA’s who are all so attentive, our Podiatry Dr Escandar who speaks to her in her Arabic language, immediately made their recommendations, setting her program and goals, she’s now on day 15 her legs have reduced 3 sizes and still improving she now walks up to 150 feet with her rolling walker, smiles and enjoys the activities, and social interactions she’s been getting she enjoys the food which our dining team tailors to her taste and adds a little flare for her culture above all I knew the team was amazingly put together by our Administrator James but watching them in action from the family side line has been such a site to witness and my husband and I are so grateful we are looking forward to her being home for Christmas #TeamChathamHills #RealResults
My Mom has been at the Chatham Hills facility since August.
It was extremely difficult to accept the fact that we might not be able to provide the proper care of Mom at home and to ensure her safety and to avoid falls.
On behalf of my family, I want to thank all staff for the care and support they have been providing for the last three months while caring for my Mom who doesn't speak English and is forgetful:
Alex - for promptly addressing all our concerns related to Mom's care and comfort; and for being our dedicated volunteer interpreter :)
James - for always finding time on his busy calendar to answers our questions and even to support me by sharing his personal experiences of being a caretaker of elderly parents.
Janelly - the Rock! This person must be cloned. I've seen Janelly, the director of nursing, serving dinners in rooms, changing patients, helping them with toileting, consoling family members etc, etc. Janelly is a unique, genuine, very wise person, and a truly compassionate professional.
Tamara - for understanding the slightest cues in my Mom's movements and what they could mean, and for attending to her with care and calling her "Babushka" (Grandma in Russian)
Fumi- for always smiling to my Mom while assisting her with hygienic tasks and singing while rubbing cream on Mom's feet. I have total peace of mind when it is Fumi's shift!
Mary, Christine, Mr. Sharif, Robert, Marleen, Farouk, Mike -the Chef- , Leo, Tanya and many other kind and patient staff - thank you for your care of Mom.
So why only 4 stars?
By all means, a few things could be improved to make the facility a high class one and a desirable destination for health improvement and long-term care. The important one is Physical Therapy. And there are times when communication could have been better and when staffing seems stretched, which affects response time and consistency. Still, when we raised concerns, most issues were addressed.
And so, this Thanksgiving season, we are grateful for the people who genuinely care of our Mom at the Chatham Hills center.
