    So I first have to thank Brandon Cruz for getting me into Aviate very easily from a hospital setting. When I arrived I was very apprehensive transferring from a hospital setting to a subacute facility. My nurse Shakira and my aide Shannon immediately put me at ease. They were both amazing. They took care of me for my entire 6 weeks there along with my other nurses, Jelisa and Patrick and my aide Princess Who were so amazing my entire stay. I have to thank the entire staff for treating me so good right down to the front desk people Mimi and Gloria and even the cleaning staff Tammy and Ishmael for making my room so clean and so pleasant. They truly got me to where I needed to be I just wish that the staff had more support and more personnel because they are so overworked but they manage to keep everyone happy and safe.Thank you Aviate and my therapists Niki and Mia I am home doing well God bless you all.

    This is a review of a specific CNA at Aviate. Shannon. I have been so lucky to have Shannon take care of me the months I have been at this facility. Shannon is an exemplary CNA. She has given 100% into my care. She is compassionate and supportive at all times. My safety is her number 1 priority. Any request I have for her is immediately handled to my satisfaction. Shannon is always thorough and gentle with handling my personal needs. TBH I have had some issues with some of the other CNAs here but never with her. She greats me every day with a smile on her face and makes sure I am settled and have all I need before she leaves for the day. Aviate is fortunate to have Shannon on their team.

    Where do I start. My husband has been at Aviate since April. He came for therapy after being contracted for several months after 30 days therapy was stopped due to no progress(after several months of contraction). I was told it was insurance.(???) The Nursing staff may need to be recertified or retrained. . Staffing starts at the top, lead by example. All need to be on the same level. I've asked the DON on several occasions for feedback on some concerns what I'm always told "I'll look into it". Never a followup conversation.

    When a nurse has to work an entire floor alone and there's a DON and Charge Nurse in the building where does the teamwork come in?

    To name a few Rochelle , Kamesha, Eugene, Shakeria, Jelisa and Sebastien are the few that stand out to me. They take their jobs to the next level. On a given but rare day when they are on at the same time, you know its going to be a good day.

    WHERE IS THE PROFESSIONALISM IN THE ADMINISTRATION AND STAFFING. (HELP ME UNDERSTAND)

    Fabulous staff nurses Shakira …Jalisa and Patrick. CNA Shannon was great.

    Therapy Jasper is the best ❤️Mia and Clare were great

    Facilities are clean and always. I do recommend Aviate.

    Update there has been various improvements with timely responses.
    The dayshift speedy response to my spouses incident was great. The Day Shift Staff kept me updated on his status as they attended to his needs.

    Please continue to move in the right direction. Increase staff seem to help control the care of residents. I will continue to monitor your improvement processes.

    Thank you for addressing my concerns.
